Documents Required: 

1. A letter of complaint
2. The complaint should be filed by the person concerned or an authorized person with a certified proxy issued by the notary public
3. All original supporting documents should be attached

Procedures:

1. The letter of complaint and all supporting documents should be submitted
2. The documents will be reviewed by the Computer Section for clarity
3. Statement of the complainant will be written down
4. Details of the person accused shall be circulated
